Nurse - Per Diem Wednesday NOC Shift

Casa de las Campanas is seeking a talented Registered Nurse with charge experience to join our team. As a Registered Nurse, you will be responsible for managing and coordinating resident care from admission through discharge within our skilled nursing facility and occasionally, residential living. You will collaborate with other interdisciplinary health care team members, including residents and responsible parties, and delegate responsibilities to nursing staff.
Key Responsibilities:- Assess residents, identify problems, plan, implement, and evaluate nursing interventions, reviewing and revising resident care plans as needed.
- Provide direct therapeutic, restorative, and preventive care to residents with identified health needs.
- Provide direction, supervision, and evaluation of unlicensed and licensed personnel in accordance with the policies and procedures of the facility.
- Admit, transfer, and discharge residents in coordination with other disciplines, interact with discharge planners, and request needed information from hospitals and responsible parties for admissions.
- Coordinate daily assignments to ensure the delivery of cost-effective, quality resident care, and ensure consistency of resident care through shift-to-shift rounds, every two-hour rounds, change of condition reports, and follow-up on doctor and family calls.
- Coordinate nursing services with facility and community resources, receive, document, and implement physician's orders, and order and administer medications, perform treatments, schedule tests, and other diagnostic work as ordered by physicians.
- Assure that residents receive the prescribed diets, report resident weight variations, and implement nutritional interventions as appropriate.
- Monitor and ensure completion of all medical record documentation, audits, and infection control log, and document weekly assessments focusing on care plan goal achievements following the facility schedule.
- Current licensure by the California Board of Registered Nursing.
- Current CPR certification.
- More than two years experience as a registered nurse in LTC with prior charge nurse or supervisory experience preferred.

Shift: 7:00 pm - 7:00 am
Wednesday Availability Required
Shift differential for PM ($1.25/hr.) and NOC ($1.50/hr.)
All Employees must be able to pass background checks (fitness for duty physical, fingerprinting, employment references) as required by a licensed residential care facility.
