The Nurse Practitioner supports the vision of the Patient-Centered Medical Home model and plays a key role in delivering high-quality, patient-centered care. This includes performing physical examinations, treating common episodic and chronic health care problems, and providing health education and counseling to patients.
The Nurse Practitioner is responsible for triage, including cross-coverage and management of clinical team messages, prescription refills, lab orders, and referral requests. They consistently project a professional and courteous demeanor and may schedule same-day acute patients as identified via the practice triage protocol.
Responsibilities- Perform physical examinations and treat common episodic and chronic health care problems
- Provide health education and counseling to patients
- Manage clinical team messages, prescription refills, lab orders, and referral requests
- Schedule same-day acute patients as identified via the practice triage protocol
- Minimum of one (1) year recent related experience as Nurse Practitioner
- Current national certification from an accepted certifying agency
- California Driver's license (CDL) if required for position
- Active RN and NP and Furnishing license in California
- BRN recognized graduate master's degree program as a nurse practitioner
- Active and/or transferable DEA license to California
- Enrollment with CMS Medicare and Medicaid through credentialing
- AHA BLS certification
