About the Role

We are seeking a highly skilled and compassionate Hospice Registered Nurse (RN) Case Manager to join our team at Resource 1 Homecare Staffing, LLC. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality, patient-centered care to individuals and families in need of hospice services.

Responsibilities
  • Conduct comprehensive assessments of patients and families to determine their unique needs and develop individualized care plans.
  • Provide expert nursing care, including pain and symptom management, to patients in their homes and communities.
  • Collaborate with the interdisciplinary team to coordinate care and ensure seamless transitions between healthcare settings.
  • Communicate effectively with patients, families, and healthcare providers to ensure that care is aligned with patient goals and preferences.
  • Participate in on-call duties and provide 24/7 support to patients and families as needed.
  • Stay up-to-date with the latest developments in hospice care and participate in ongoing education and training to enhance your skills and knowledge.
Requirements
  • Graduation from an accredited school of nursing with a current RN license.
  • 1-2 years of recent acute care experience in an institutional setting, with a preference for home care experience.
  • Valid driver's license and insured automobile.
  • Ability to work efficiently with minimal supervision and build strong relationships with patients and families.
  • Passion for hospice care and commitment to delivering high-quality, patient-centered care.
