Geneva Lake Manor is a respected non-profit provider of short-term rehabilitation and long-term care, known for delivering quality services to seniors in our community for over 40 years.
We are currently seeking a full-time Registered Nurse (RN) to join our team as a Nurse Manager for Transitional Care Services. The ideal candidate will possess strong clinical and leadership abilities, prioritizing person-centered care while establishing trust and fostering lasting relationships with residents and their families.

Key Responsibilities:
- Review and assess incoming referrals
- Communicate effectively with hospitals and facility teams regarding admissions
- Input referrals into the electronic medical record (EMR)
- Welcome residents and oversee the admission process
- Maintain strong relationships with local hospitals
- Promote available beds to hospitals and the community
- Ensure all referrals are tracked and followed up on
- Conduct assessments for new admissions
- Assist on the floor as needed
- Update admission documentation as required

Position Requirements:
- Graduate of an accredited nursing program
- Current state licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in good standing
- CPR certification
Preferred Qualifications:
- Experience in long-term care as a Registered Nurse or Licensed Practical Nurse is preferred.
