Job Summary

As a Behavioral Health Clinician at Atlantic Health System, you will play a vital role in coordinating the psychosocial needs of patients in conjunction with our interdisciplinary treatment team. Your expertise will be instrumental in completing psychosocial histories, supplementing additional information to our database, and implementing and facilitating treatment plans. You will identify barriers to learning and address patient educational needs, employing appropriate one-to-one patient and family contact as needed. Your therapeutic groups will be tailored to the inpatient setting, and you will consistently document, implement, and facilitate aftercare plans. You will provide patients and families with information on post-hospital options, obtain necessary consent for referral processes, and arrange appointments with aftercare facilities or individual providers. Additionally, you will initiate transfers and relate guidelines for transfer to nursing staff, and facilitate the conversion of voluntary status to involuntary commitment, coordinating with appropriate Social Service agencies for placement in alternate facilities.

Qualifications
  • Master's Degree in Social Work (MSW) required.
  • Current LSW or equivalent LCSW preferred.
