Care Coordinator, American Health Partners

About the Role:
We are seeking a highly skilled Care Coordinator to join our team at American Health Partners. As a Care Coordinator, you will play a vital role in coordinating customer service and support to nursing facilities and behavioral healthcare centers, ensuring accurate and timely delivery of medications and supplies.
Key Responsibilities:
- Act as liaison between input, dispensing, and inventory departments to facilitate effective delivery of pharmacy orders and quality assurance of all pharmacy procedures.
- Generate daily cycle fill orders for daily delivery to facilities.
- Manage and resolve incoming requests from facilities concerning pharmacy orders.
- Track and communicate with nursing facilities concerning equipment availability and delivery.
- Responsible for obtaining medication prescriptions for scheduled medications.
- Refer specific clinical questions to pharmacists.
- Manage medical records for selected facilities, including monthly recertifications and MARs.
- Notify consultant pharmacist of state surveys at facilities and assist with questions.
- Conduct daily Visions/Framework audits.
- Prepare documentation and assist external auditors for Medicare Part D audits.
- Maintain Pharmacy Issue Logs and research and resolve applicable issues.
- Generate and submit daily narcotic reporting as required by the Tennessee Board of Pharmacy.
- Research, manage, and track non-formulary medications and generate prior authorizations and medication change requests with guidance from pharmacists.
- Perform input and dispensing pharmacy technician duties as needed.
- Communicate information effectively to supervisors and others in a timely manner.
- Maintain proficiency with computer systems.
- Demonstrate accuracy and thoroughness and continually strive to improve quality standards.
- Maintain confidentiality of work-related information and materials in compliance with HIPAA regulations.
- Adhere to the principles of the Employee Standards and Code of Conduct and report any violations.
- Knowledgeable and compliant with current company policies and procedures and state and federal regulations.
- Attend meetings and in-services as required.
Requirements:
- Three years of pharmacy technician and/or other clinical-related experience.
- Active Tennessee Board of Pharmacy Technician Registration.
- Pharmacy Technician Certification (CPhT) (preferred).
- High school diploma or general education degree (GED).
- Graduate of an accredited Nursing Program or Pharmacy Technician Program.
- Or equivalent combination of education and/or experience.
Working Conditions:
- Occasional lifting of objects up to 40 pounds.
- Subject to standing, walking, sitting, bending, reaching, kneeling, pushing, and pulling.
- Work is performed in a standard office environment.
- Must have reliable transportation.
