RN Case Manager

**Responsibilities:**
* Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
* Make referrals to other disciplines as indicated by the patient's identified needs or document rationale for not doing so.
* Promote patient health and independence through teaching and appropriate rehabilitative measures, assisting patients in learning appropriate self-care techniques.
* Supervise LPNs and HHAs.
* Complete documentation timely and accurately.
* Regularly communicate patient progress to the clinical manager and care team.
* Plan and provide staff education.
* Perform on-call responsibilities and on-call services to patients/families as assigned.
* Participate in clinical development and continuing education programs.
**Qualifications:**
* One (1+) year of clinical experience as a Registered Nurse (RN). If less than 1 year clinical experience as a RN, candidate must be approved by VP Clinical.
* Current RN license, specific to the state(s) you are assigned to work.
* Current CPR certification.
* Valid driver's license, reliable transportation and liability insurance.
