LPN / Licensed Practical Nurse - Compassionate Caregiver

As a Licensed Practical Nurse, you'll play a vital role in our team, delivering compassionate care and promoting independence in a variety of settings.

Responsibilities:
Provide one-on-one care to clients, following their personalized plans of care and maintaining their dignity and autonomy.
Monitor vital signs and report any abnormalities to the case manager and physician, ensuring timely interventions and assessments.
Administer medications, feedings, oxygen, and ostomy care as needed, documenting results and communicating with family, case managers, and physicians.
Collaborate with other care professionals, such as physical therapists, occupational therapists, and speech therapists, to establish routines and ensure comprehensive care.
Maintain infection control policies, reporting possible communicable diseases or infections, and ensuring the implementation of infection control procedures.
Accurately document nursing actions, communicate with family and case managers, and document progress and outcomes for established goals.
Order supplies, medications, and feedings as needed, ensuring adequate stock levels and meeting client needs.
Ensure all patient needs are met in accordance with physician instructions, monitoring incidents and completing timely, accurate incident reports.
Maintain medical equipment, reporting faulty equipment to Durable Medical Equipment for repair.

Qualifications:
Current license as a Licensed Practical Nurse or Licensed Vocational Nurse in good standing from the state(s) of practice.
Current Cardiopulmonary resuscitation certification.
Ability to work in a home-like setting, bending, stooping, pushing, pulling, sitting, and walking for periods of time.
Knowledge of individual service plans, including understanding of client rights and health needs.
Must have completed post-secondary coursework to facilitate licensure as a Licensed Practical Nurse or Licensed Vocational Nurse.
