Job Summary

As a Licensed Practical Nurse (LPN) at LifeBridge Health, you will play a vital role in providing high-quality care to our residents in a long-term care setting. Under the supervision of the Registered Nurse/Director of Nursing, you will utilize your skills and knowledge to develop and implement individualized nursing care plans that meet the unique needs of each resident.

You will be responsible for attending to the daily operations of the unit, providing leadership as needed, and working collaboratively with the healthcare team to ensure the best possible outcomes for our residents.

Responsibilities
  • Develop and implement individualized nursing care plans for residents
  • Provide direct patient care, including administering medications, performing vital signs, and providing wound care
  • Assist with daily operations of the unit, including maintaining a clean and safe environment
  • Collaborate with the healthcare team to ensure seamless care transitions
  • Participate in quality improvement initiatives to enhance resident care
Requirements

To be successful in this role, you will need:

  • A current LPN license in the state of Maryland
  • Basic computer skills, including proficiency in electronic medical records
  • Ability to work effectively in a fast-paced environment
  • Strong communication and interpersonal skills
  • Ability to lift 25 pounds and stand for long periods
