Join Our Team as a Physical Therapist for Home Health Visits

BAYADA Home Health Care is seeking an experienced Physical Therapist to join our team in Harrisburg, PA. As a home health physical therapist, you will be an integral member of a multidisciplinary healthcare team providing rehabilitative care and skilled nursing to clients in their homes.

Responsibilities:
  • Conduct home visits to clients in designated territories, assessing their physical needs and developing individualized care plans.
  • Perform diagnostic tests and measurements, such as mobility and range of motion assessments, to identify areas for improvement.
  • Develop and implement manual therapeutic exercises, gait training, and other interventions to promote client independence and safety.
  • Collaborate with nursing staff to revise care plans and ensure seamless care coordination.
  • Educate clients and their families on rehabilitative care and activities necessary for optimal health and safety.
  • Accurately document client observations, interventions, and evaluations using a state-of-the-art touch pad tablet.
Qualifications:
  • A current Pennsylvania Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills, with prior experience with electronic medical records (EMR) preferred.
