Job Title: Case Manager
Job Summary:
Services For The Underserved, Inc. is seeking a skilled Case Manager to join our team. As a Case Manager, you will provide comprehensive case management services to clients in our homeless prevention program. Your primary responsibility will be to conduct intake processes, provide documentation, and facilitate group and individual counseling sessions. You will also conduct room inspections, provide referrals to medical, mental health, and social services, and assist with crisis intervention.
Responsibilities:
* Conduct intake processes within 24 hours of admission
* Provide accurate and timely documentation of client services
* Facilitate group and individual counseling sessions
* Conduct room inspections to assess client ADL skills
* Provide referrals to medical, mental health, and social services
* Assist with crisis intervention
Requirements:
* High School Diploma required
* Two years' experience working with homeless populations
* Ability to work with culturally diverse populations
Preferred Qualifications:
* Bachelor's Degree in Human Services field
* Familiarity with NYC public school system
* Experience in residential settings, group work, tenant organization, case management, referral, and community outreach
