About the Role

We are seeking an experienced Comprehensive Adult Support Specialist to join our team at Polara Health. This is a rewarding opportunity for a skilled professional to make a meaningful difference in the lives of adults with complex needs.

Job Summary

The successful candidate will be responsible for providing comprehensive support services to adults in their community, working closely with other members of the healthcare team to ensure seamless care coordination. This role requires strong clinical skills, excellent communication abilities, and a passion for delivering high-quality patient outcomes.

Key Responsibilities
  • Community Support Services: Provide regular visits to clients in their homes, conducting assessments and developing personalized support plans to meet their unique needs.
  • Clinical Assessments: Conduct comprehensive clinical assessments to identify mental health needs and develop effective treatment plans.
  • Therapeutic Interventions: Deliver short-term therapeutic interventions as needed, working collaboratively with clients to achieve their goals.
  • Crisis Management: Manage crisis situations effectively, providing stabilization and support to clients in times of need.
  • Team Collaboration: Work closely with other healthcare professionals to ensure integrated and holistic care, coordinating efforts to achieve optimal patient outcomes.
Requirements
  • Education: Master's degree in a related field (counseling, social work) or licensed/eligible.
  • Experience: 2-4 years of experience working within behavioral health, with a focus on adult services.
  • Clinical Skills: Valid driver's license, critical thinking/problem-solving skills, and ability to delegate and lead a clinical team.
  • Knowledge: Understanding of DSM 5, with a commitment to staying up-to-date on industry developments.
What We Offer

We offer a competitive salary range of $65,000 - $80,000 per year, depending on experience.
