**Job Summary:**
Constellation Health Services is seeking a dedicated and compassionate Hospice Registered Nurse (RN) to provide comprehensive care to patients facing end-of-life challenges. As a Hospice RN Case Manager, you will work closely with our interdisciplinary team to assess patient needs, develop personalized care plans, and provide emotional support to patients and their families.
**Responsibilities:**
* Deliver high-quality nursing care to patients with complex needs
* Conduct thorough assessments to identify patient needs and develop individualized care plans
* Manage medications and treatments as prescribed by the physician
* Provide emotional support and guidance to patients and their families
* Educate patients and families about hospice care and end-of-life issues
* Collaborate effectively with interdisciplinary team members to ensure coordinated care
* Maintain accurate and up-to-date patient records
**Qualifications:**
* Current and active RN license in the state of employment
* Bachelor's degree in nursing (BSN) preferred
* Minimum of 1 year of experience in hospice care or end-of-life care setting
* In-depth understanding of end-of-life care principles and practices
* Exceptional communication and interpersonal skills
* Ability to work independently and as part of a team
