**Job Title:** Care Coordinator Specialist

Carastar Health is seeking a highly motivated and compassionate Care Coordinator Specialist to join our team. In this role, you will provide critical support services to individuals with Serious Mental Illness/Serious Emotional Disturbance (SMI/SED).

As a Care Coordinator Specialist, you will be responsible for facilitating collaborative relationships among youth, their families, and involved child-serving systems. This includes providing case management services, needs assessments, service arrangements, social supports, and monitoring.

Key Responsibilities:

  • Provide initial contact with recipients and families within 24 hours of receiving a referral for services.
  • Offer Low Intensity Care Coordination (LICC) services and schedule an intake interview within seven days.
  • Conduct comprehensive home-based assessments (CANS) within 14 days of acceptance of LICC services.
  • Develop Crisis Stabilization & Support Plans to ensure safety and minimize risk of out-of-home placement in moments of crisis.
  • Provide crisis intervention as needed to de-escalate potential crisis events and support the recipient in remaining in-the-home.
  • Make referrals and assist with scheduling appointments for the recipient.
  • Evaluate recipient progress towards accomplishing goals listed in the LICC Care Plan at intervals of six months or less.

Requirements:

  • Bachelor's degree in behavioral science or related field from a recognized college or university.
  • Must meet requirements for approved Medicaid Provider status.
  • Valid driver's license and acceptable driving record.
  • Liability coverage on personal vehicles.

Salary: $60,000 - $80,000 per year, depending on experience.
