About the Role

We are seeking a compassionate and skilled Hospice Care Specialist to join our team. As a key member of our hospice care team, you will be responsible for providing high-quality patient care and support to individuals with serious illnesses or terminal diagnoses.

Salary and Benefits

This full-time position comes with a competitive salary of approximately $83,000 per year, plus benefits, including medical, dental, and vision plans, wellness programs, paid time off, and more.

Job Responsibilities

  • Provide direct patient care services, including assessing patient needs and developing personalized care plans
  • Collaborate with interdisciplinary teams to ensure seamless care coordination
  • Empower patients' caregivers through education and support
  • Maintain accurate patient records and participate in quality improvement initiatives

Requirements and Qualifications

To be successful in this role, you should possess:1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology settings

RN license in the state of employment (or eligibility to obtain)Demonstrated ability to work effectively in a fast-paced environment and adapt to changing situations
