Registered Dietitian

We are seeking a skilled Registered Dietitian to join our team at Elderwood at Ticonderoga. As a Registered Dietitian, you will play a vital role in ensuring the health and well-being of our residents by providing nutrition assessments, developing individualized care plans, and conducting routine audits.

• Conduct comprehensive nutrition assessments for adult and geriatric residents in a long-term care and sub-acute rehabilitation setting.
• Develop, implement, and monitor individualized nutrition care plans based on current best practices.
• Provide nutrition counseling and education to residents and their family members.
• Maintain accurate and up-to-date resident nutrition information in our computer system.
• Conduct routine audits and monitor performance improvement initiatives related to nutrition and food service.
• Participate in resident care plan meetings, staff meetings, and other relevant programs.
• Stay current with federal and state regulations regarding nutritional services in long-term care settings.

• Bachelor of Science degree or higher in Nutrition/Dietetics from an accredited program.
• Successful completion of an internship or equivalent clinical management practice.
• Active and ongoing registration with the Commission on Dietetic Registration.
• Active membership in The Academy of Nutrition and Dietetics (preferred).
• Certified Dietitian - Nutritionist (CDN) with The New York State Department of Education or licensure in the state of employment (or to be obtained within one year from date of hire).
