Nutrition Services OverviewAt Elderwood, we are seeking a skilled Registered Dietitian to join our team and contribute to the health and well-being of our residents. As a Registered Dietitian, you will be responsible for conducting nutrition assessments, developing individualized nutrition care plans, and providing nutrition counseling and education to residents and their families.
Responsibilities:
- Conduct nutrition assessments for adult and geriatric populations in a long-term care and sub-acute rehabilitation setting.
- Develop, implement, and monitor individualized nutrition care plans based on current best practices.
- Provide nutrition counseling and education to residents and their families.
- Maintain resident nutrition information in the computer system.
- Conduct routine audits and monitor performance improvement initiatives related to nutrition and food service.
- Attend and participate in resident care plan meetings, staff meetings, and other meetings as required.
Qualifications:
- Bachelor of Science degree or higher in Nutrition/Dietetics from an accredited educational program.
- Successful completion of an internship or equivalent clinical management practice qualifying for Professional Registration with the Commission on Dietetic Registration.
- Active and ongoing Registration with the Commission on Dietetic Registration.
- Active membership in The Academy of Nutrition and Dietetics preferred.
- Certified Dietitian - Nutritionist (CDN) with The New York State Department of Education or Licensure in the state of employment (or to be obtained within one year from date of hire).
