Job Summary

We are seeking a skilled Licensed Practical Nurse to join our team at The Sharon at SouthPark. As a LPN, you will play a vital role in providing high-quality care to our residents, working closely with our interdisciplinary team to ensure the best possible outcomes.

Key Responsibilities
  • Assess, implement, and document resident care as ordered by a physician and with the continuous availability of a registered nurse.
  • Work with the physician during outpatient clinic hours, obtaining vital signs, chief complaint, medication reconciliation, and ensuring that orders are carried out accurately.
  • Schedule appointments, follow-up appointments, and call patients for reminders.
  • Provide education to residents and families regarding medications, disease processes, and treatment recommendations.
  • Document and process physician orders, as well as process and follow up on referrals to outside specialists and therapies.
  • Perform assigned duties in the Wellness Clinic, prepare and administer medications and treatments as ordered by physician, and document appropriately.
  • Participate in on-going resident assessment and evaluate resident's response to nursing actions under the guidance of a registered nurse.
  • Document changes in resident conditions and notify physician, responsible party, and supervising RN of changes.
  • Keep physicians updated on resident's condition as needed.
  • Ensure charges for nursing services and supplies are billed accurately.
  • Participate in the development, implementation, and review of service plans with the supervision of the Registered Nurse.
  • Comply with company policies, procedures, and related local, state, and federal regulations.
Requirements
  • Ability to provide nursing care for residents requiring minimal to total care as defined in the health care plan established by an RN.
  • Ability to communicate effectively, orally and in writing.
  • Ability to work with necessary medical equipment.
  • Ability to function effectively within a team environment.
  • Ability to deal effectively with confused and/or agitated residents.
Education and Experience

Candidates for this position must be a graduate of an accredited school of nursing and possess, at a minimum, the earned title of Licensed Practical Nurse. Current unencumbered North Carolina Licensed Practical Nurse license is required.
