Job Summary

We are seeking a skilled and compassionate Registered Nurse Case Manager to join our team. As a RN Case Manager, you will play a critical role in coordinating patient care and ensuring that our patients receive the highest level of care and support.

Key Responsibilities
  • Coordinate patient care and develop individualized care plans to meet the unique needs of each patient.
  • Conduct thorough assessments of patients' medical and social needs, and develop plans to address these needs.
  • Collaborate with healthcare providers, family members, and other stakeholders to ensure that patients receive comprehensive and coordinated care.
  • Provide education and support to patients and their families to promote health and wellness.
  • Monitor patient progress and adjust care plans as needed to ensure optimal outcomes.
Requirements
  • Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation.
  • One (1) year experience as a RN.
  • Licensed to practice as a registered nurse in the state of agency operation.
  • Must possess and maintain valid CPR certification while employed in a clinical role.
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. Ability to travel to all business locations.
