Key Responsibilities- Coordinate patient care and develop individualized care plans to meet the unique needs of each patient.
- Conduct thorough assessments of patients' medical and social needs, and develop plans to address these needs.
- Collaborate with healthcare providers, family members, and other stakeholders to ensure that patients receive comprehensive and coordinated care.
- Provide education and support to patients and their families to promote health and wellness.
- Monitor patient progress and adjust care plans as needed to ensure optimal outcomes.
- Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation.
- One (1) year experience as a RN.
- Licensed to practice as a registered nurse in the state of agency operation.
- Must possess and maintain valid CPR certification while employed in a clinical role.
- Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. Ability to travel to all business locations.
