We are seeking a compassionate and skilled Registered Nurse to join our team as an Afterhours Palliative Care RN at Illumia. As a member of our interdisciplinary team, you will play a vital role in providing high-quality patient care and support to individuals and their families facing advanced illnesses.
Key Responsibilities- Provide preventive and supportive care to palliative patients and families, utilizing assessment, intervention, and teaching skills to maximize patient comfort and enhance quality of life.
- Assume afterhours coverage for designated teams and function as an interdisciplinary advanced illness management and support, including telephonic, video, and in-person interventions.
- Collaborate with patients, families, attending physicians, and other members of the IDG to provide patient and family care.
- Timely document patient problems, goals, interventions, and patient/family response to palliative care.
- Communicate effectively with members of the AIM team, clinical supervisor, and patient's PCP regarding changes in patient status and/or patient needs occurring during afterhours calls.
- Associates/Bachelor's degree in Nursing.
- 2 years of prior skilled Registered Nurse experience.
- Demonstrated ability to provide quality patient care.
- Nursing experience in palliative care, oncology, hospice, or geriatric care preferred.
