Responsibilities:
- Conduct thorough social services evaluations and provide referrals as needed.
- Assist guests with financial and legal matters, such as applying for pensions or referrals to lawyers.
- Develop and implement discharge plans to ensure a safe and successful transition for our guests.
- Provide counseling services and psychiatric medication management as indicated.
- Maintain accurate and timely documentation in MDS care plans.
Requirements:
- Current social work licensure in the state.
- Bachelor's degree in social work required; master's degree preferred.
- One year of supervised social work experience in a healthcare setting is preferred.
