Hospice Nurse Clinician

We are seeking a compassionate and skilled Hospice Nurse Clinician to join our team at Providence Home and Community Care. As a key member of our interdisciplinary team, you will provide holistic care to terminally ill patients and their families, assessing their needs and developing personalized plans to enhance their quality of life.
Responsibilities- Assess patient needs and develop comprehensive care plans
- Collaborate with the IDG to anticipate changes in patient condition and prevent crises
- Provide education and support to patients and families
- Work with the team to ensure continuity of care and promote patient choice
- Associate's or Bachelor's Degree in Nursing
- Washington Registered Nurse License
- 1 year of nursing experience, with some pediatric experience preferred
