LPN/LVN Hospice Care Specialist

We are seeking a compassionate and skilled LPN/LVN to join our hospice care team as a Nurse Clinician. In this role, you will provide holistic care to terminally ill patients and their families, working collaboratively with our interdisciplinary team to anticipate changes in patient condition, avoid crises, and promote patient and family choice.
Key Responsibilities- Provide nursing care according to the Plan of Care under the supervision of a registered nurse.
- Work collaboratively with the IDG to promote patient and family choice, assure continuity of care, and enhance the quality of life of dying patients.
- Develop and implement individualized care plans that address the physical, emotional, and spiritual needs of patients and their families.
- Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
- Current Washington Licensed Practical Nurse (LPN) licensure.
- 1 year of nursing experience, preferably in hospice, home care, or long-term care.
- National Provider BLS certification within 30 days of hire.
