Job Opportunity

Ambercare is seeking a skilled Licensed Clinical Social Worker to join our hospice team in Santa Fe. This is a full-time position offering a competitive salary of up to $61,000 annually, mileage reimbursement, and a 401(k) match.

Qualifications:

  • Education: Master's Degree in Social Work (MSW) required for New Mexico residents.
  • Licensure: Current State Licensure in Social Work (LMSW or LCSW) required.
  • Experience: Minimum of one year's experience in healthcare or hospice settings.
  • Other: Valid driver's license and proof of insurance are necessary.

Responsibilities:

  • Perform psychosocial assessments of patients and families/caregivers in a timely manner, addressing identified needs in the patient's plan of care.
  • Demonstrate exceptional service to patients, families, referral sources, co-workers, and other customers, assisting team members as needed.
  • Counsel patients and families/caregivers and staff as needed regarding stress and other coping difficulties.
  • Provide accurate and timely documentation of patient services to reflect the plan of care.
  • Assist with applications for Supplemental Security Income, Hospice, Medicaid, and Medicaid Waiver, obtaining resources from community social, health, pharmaceutical, and welfare agencies to meet patient/family needs.
  • Serve as a liaison between patients and families/caregivers and community agencies.
