Ambercare Hospice Social Worker Job Description

We are seeking a compassionate and skilled Licensed Clinical Social Worker (LCSW) to join our hospice team in Santa Fe, New Mexico. As a key member of our interdisciplinary team, you will provide high-quality psychosocial support to patients and their families, ensuring their physical, emotional, and spiritual needs are met.

Responsibilities:
  • Conduct thorough psychosocial assessments of patients and families to identify their needs and develop personalized care plans.
  • Provide counseling and support to patients and their loved ones, addressing stress, coping difficulties, and other challenges.
  • Collaborate with healthcare professionals to ensure seamless care coordination and communication.
  • Document patient services and progress accurately and timely, reflecting the care plan.
  • Assist patients and families with applications for benefits, such as Supplemental Security Income, Medicaid, and Medicaid Waiver.
  • Serve as a liaison between patients, families, and community agencies, connecting them with resources and services.
Requirements:
  • Master's Degree in Social Work (MSW) from an accredited program.
  • New Mexico State Licensure in Social Work (LMSW or LCSW).
  • Minimum one year of experience in healthcare or hospice settings.
  • Valid driver's license and proof of insurance.
