Acts as a patient advocate, intermediary, and advisor/educator to assist individuals in navigating the healthcare continuum.

Work Schedule:

Full Time Days Responsibilities:
Conducts regular rounds on assigned patients and assesses their progress in relation to the care plan. Communicates the care plan to patients and their families, addressing any concerns, questions, or issues that arise.

Provides emotional support, counseling, clinical education, and expert guidance to patients and families, empowering them to understand and actively participate in their healthcare journey and personal decision-making.

Serves as a bridge between patients and clinical staff, administration, physicians, managed care organizations, and community resources to optimize resources and identify/resolve barriers to the care plan.

Assists patients and families in addressing financial, psycho-social, functional, and administrative challenges by providing options and referring them to suitable resources.

Advocates for patients and families as needed.
Requires critical thinking skills, decisive judgment, and the ability to work independently within the scope of the role. Must thrive in a fast-paced environment and take appropriate actions.

Qualifications:

Minimum Required Education:
Graduation from an accredited or state board of nursing approved Registered/Professional Nursing program if less than 3 years of experience.

Preferred:
Bachelor's degree in Nursing.

Required Licensure/Certification:
State licensure as a Registered Nurse (RN). Relevant life support certification as determined by position level.

Minimum Experience:
2 years of nursing experience.

BLS certification through the American Heart Association or the American Red Cross CPR for the Professional Rescuer must be obtained before the start date.

BLS or CPR certification must remain valid for at least sixty days post-hire.

All life support certification cards must be issued by either the American Heart Association or the American Red Cross CPR for the Professional Rescuer.

(American Red Cross for the Professional Rescuer is only valid upon hire; all renewals must be through an American Heart Association sanctioned course.)

Compensation:
The pay range for this position is:
$60.31 / hour. Compensation is based on the applicant's relevant experience.

The bonus accumulates annually each October and is disbursed in October following the completion of five years of employment.
