Discharge Planner

Job Summary:
The Discharge Planner will work in conjunction with the multidisciplinary team to plan, manage, and coordinate the discharge plan of acutely hospitalized patients. This role requires excellent communication skills, knowledge of available community resources, and the ability to work in an intense environment.
Responsibilities:
- Collaborate with the healthcare team to develop and implement discharge plans
- Coordinate with community resources to ensure a smooth transition for patients
- Communicate effectively with patients, families, and healthcare providers
- Manage and maintain accurate records of patient information
Requirements:
- Bachelor's degree in a related field or equivalent experience
- Current valid California LVN or RN license or education in healthcare administration
- Previous experience in discharge planning or a related field
- Knowledge of Medicare/CMS guidelines, Erickson's Life Skills, and TJC standards
Working Conditions:
