About This Role

We are looking for an outstanding full-time Facility Liaison to join our field team providing assistance to our Physicians and Mid-Level Providers in skilled nursing and assisted living facilities.

Responsibilities:

  • Mobile travel to facilities
  • Interview and verify patient information and record medical history, confirming purpose of visit
  • Prepare patients for examination by performing vitals and reporting patient history summary
  • Secure patient information and maintain patient confidentiality by HIPPA Compliance
  • Counsel patient or Facility Staff by transmitting physician's orders
  • Coordinating the daily administrative activities, including scheduling, prepare EMR charts, medicine reconciliation, and consent forms
  • Maintain safe, secure, and healthy work environment by establishing and following standards and procedures, complying with legal regulations
  • Enhance practice reputation by accepting ownership for accomplishing new and different requests; exploring opportunities to add value to job accomplishments
  • Maintain patient medical records

Salary and Benefits

$55,000 - $65,000 per year, depending on experience. Our benefits package includes health insurance, retirement plan, paid time off, and professional development opportunities.
