Job Summary

We are seeking a skilled Clinical Care Coordinator to join our Case Management team at the University of New Mexico Hospitals. In this role, you will be responsible for coordinating patient care and ensuring quality, cost-effective outcomes.

About the Role
  • Coordinate patient care with physicians, nurses, and other healthcare professionals to achieve desired outcomes.
  • Develop and implement comprehensive care plans that meet individual patient needs.
  • Monitor and evaluate patient responses to treatment, making adjustments as necessary.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Collaborate with interdisciplinary teams to promote continuity of care and optimize patient outcomes.
Requirements
  • RN in NM or eligible for reciprocal agreement.
  • 2+ years of experience in clinical care coordination or related field.
  • BLS certification within 30 days of hire.
