Job Title: Home Health Physical Therapy Assistant

LHC Group is seeking a skilled Physical Therapy Assistant to join our team in providing high-quality care to patients in their homes. As a Physical Therapy Assistant, you will work under the supervision of a Physical Therapist to deliver evidence-based treatment and education to patients, promoting their overall well-being and improving their functional abilities.

Responsibilities:
  • Deliver treatment and exercise instruction to patients in their homes, following the established plan of care.
  • Work under the supervision of a Physical Therapist to ensure adherence to state-specific laws governing physical therapy in home care.
  • Perform procedures within the scope of the physical therapy assistant and according to state laws governing physical therapy assistants.
  • Observe, record, and report patient responses to treatment and changes in their condition to the supervising Physical Therapist, nurse supervisor, and/or physician.
  • Coordinate care with other healthcare team members as needed.
  • Instruct patients, families, caregivers, and other healthcare team members in therapy areas within the scope of the physical therapy assistant.
Requirements:
  • Current licensed, registered, or certified to assist in the practice of physical therapy in the state of practice.
  • Current CPR certification.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
