**Job Summary:**

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Constellation Health Service. As a Per Diem LPN, you will provide end-of-life care and hospice services to patients in their home or community setting.

**Responsibilities:**

  • Provide nursing care as defined in the State Nurse Practice Act, including specialized duties related to end-of-life care and home care.
  • Implement current nursing practice following a comprehensive assessment and the plan of care.
  • Maintain accurate and timely documentation of patient services to reflect the plan of care.
  • Assess and provide patient and family/caregiver education and information pertinent to diagnosis and plan of care.
  • Recognize and interpret symptoms and institute remedial measures within the scope and limits of a Licensed Practical Nurse.
  • Observe, record, and report to the immediate supervisor any changes in the patient's condition.
  • Participate in the coordination of hospice and other home care services, appropriately reporting identified needs to the interdisciplinary group.
  • Participate in IDT meetings.
  • Implement care to achieve outcomes of comfortable and safe dying and effective grieving for the patient and family.
  • Use and prepare equipment and supplies effectively, adhering to aseptic technique.
  • Provide appropriate pain/symptom management.
  • Evaluate the patient's response to treatments/medications.
  • Perform other duties as assigned by the registered nurse or clinical supervisor.

**Requirements:**

  • Graduate of an accredited practical nurse program.
  • Connecticut State Licensed LPN with a minimum of 1 year of clinical experience.
  • Community home health care/hospice experience is preferred.
  • Clinical auditing background preferred.
  • Experience conducting chart reviews preferred.
  • Wound care experience preferred.
