Job Description:
The Case Manager (Discharge Planner) is responsible for coordinating healthcare decisions using a systematic approach to improve quality and outcomes. They coordinate care across the continuum, promote cost-effective care, and implement safe discharge plans.
Responsibilities:
- Clinical Interventions/Discharge and Care Planning Management
- Fiscal Management
- Payer/Referral Management
Qualifications:
Current licensure in Social Work or a clinical discipline (RN preferred) per state guidelines. Previous experience in case management and discharge planning is preferred. CCM Certification is also desirable.
