Job Summary

Exalt Health Rehabilitation Hospital Conroe is seeking a skilled Pharmacist (PRN) to join our interdisciplinary team. As a vital member of our inpatient rehabilitation hospital, you will play a crucial role in ensuring the safe and effective use of medications to optimize patient outcomes.

Key Responsibilities
  • Medication Order Verification: Review and verify medication orders for accuracy, appropriateness, potential drug interactions, and allergies.
  • Medication Dispensing: Prepare, compound, and dispense medications according to established protocols and physician orders.
  • Patient Counseling and Education: Provide clear, understandable information to patients and their caregivers regarding medication use, potential side effects, and safe administration practices.
  • Drug Therapy Monitoring: Assess patient medication regimens for safety, efficacy, and potential adverse drug reactions. Collaborate with physicians to recommend adjustments or alternatives as needed.
  • Clinical Support: Participate in multidisciplinary patient care rounds, providing medication-related expertise and recommendations to the healthcare team.
  • Inventory Management: Maintain appropriate drug inventory levels, ensuring the timely ordering of medications and supplies while overseeing the proper storage and disposal of pharmaceuticals.
  • Regulatory Compliance: Adhere to all federal, state, and local regulations governing pharmacy practice, including controlled substance management and documentation.
  • Continuous Education: Remain updated on the latest advancements in pharmacology and therapeutics through participation in professional development activities.
Requirements
  • Licenses or Certifications: Current, unencumbered license to practice as a Pharmacist in the state where currently practicing. Current BLS is required via the American Heart Association or American Red Cross.
  • Education, Training, and Experience: Doctor of Pharmacy (Pharm.D.) degree from an accredited college of pharmacy.
  • Skills and Abilities, Proficiency and Productivity Standards: Strong clinical knowledge of pharmacology and drug therapy principles. Organizes and prioritizes tasks and projects, accurately completing job responsibilities within the specified time constraints. Excellent attention to detail and accuracy. Analytical and problem-solving abilities. Ability to work under deadlines and handle multiple tasks. Speaks and communicates English effectively in verbal and written format; writes legibly. Reading and comprehension at the level necessary to perform job duties appropriately. Works cooperatively with co-workers and other hospital employees. Functions proficiently in a Microsoft Windows environment (Outlook, Word, Excel, PowerPoint).
