Responsibilities

As a Hospice RN - On-Call Specialist, you will be responsible for:

  • Providing coordination of care and ensuring continuous assessment of each patient's and family's needs
  • Implementing the interdisciplinary care plan and performing services in accordance with the physician's orders and the established plan of care (POC)
  • Ensuring delivery of care is compliant with federal and state regulations

Qualifications

To be successful in this role, you must:

  • Be a graduate of an approved school of nursing education
  • Be licensed in the state where you currently practice
  • Have at least one year experience as a licensed registered nurse (RN)
  • Have basic demonstrated technology skills, including operation of a mobile device

Preferred Qualifications

We prefer candidates with:

  • Medicare home health or hospice experience
