We are seeking a highly skilled and compassionate Registered Nurse to join our team at Bridgeville Rehab in Bridgeville, PA. As a Care Management Registered Nurse, you will play a vital role in providing exceptional patient care and collaborating with our interdisciplinary team to develop effective care plans.
Key Responsibilities:- Perform comprehensive nursing assessments and develop individualized care plans to meet the unique needs of our patients.
- Collaborate with the nursing team, physicians, and other healthcare professionals to ensure seamless care coordination and communication.
- Delegate patient care responsibilities to staff and supervise them to ensure high-quality care delivery.
- Communicate patient information effectively with assigned staff and between shifts to ensure continuity of care.
- Must be a graduate of an approved school of nursing and currently licensed by the State Board of Nursing.
- CPR Certification is required.
- One to three years of experience working as a RN in a related setting is preferred.
- Competitive hourly rate of $40 - $43 per hour.
