Community Health Practitioner Opportunity

CAPITAL AREA HEALTH NETWORK is committed to providing exceptional healthcare services to our community. We are seeking a dedicated Community Health Practitioner to join our team.

Estimated Annual Salary: $160,000 - $220,000, based on experience

About the Role: As a Community Health Practitioner, you will play a vital role in promoting health and well-being within our community. Your responsibilities will include developing and implementing health programs, conducting outreach and education, and collaborating with community partners to address health disparities.

Key Responsibilities:

  • Design and implement effective health programs to address specific community needs
  • Build strong relationships with community partners, stakeholders, and healthcare providers
  • Conduct health education and outreach activities to promote healthy behaviors and prevent disease
  • Monitor and evaluate program effectiveness, making adjustments as needed

Requirements:

  • Degree from an accredited institution in a field related to public health or community development
  • Experience working in community health settings, preferably in a leadership or management role
  • Strong communication, collaboration, and project management skills
