Qualifications
  • Graduation from an accredited nursing program is required.
  • Associate or Bachelor's degree in nursing is preferred.
  • Current valid nursing license is required.
  • One year of relevant experience is preferred.

Responsibilities

The Clinical Nurse Specialist acts as a leader within the healthcare team at PHO by establishing genuine relationships with patients and their support systems. This role involves collaborating with physicians and other team members to assess, plan, implement, and evaluate individualized care plans that promote optimal health or facilitate a peaceful end-of-life experience. The Clinical Nurse Specialist utilizes evidence-based practices and compassion to guide patients through their health journeys. They accept responsibility, authority, and accountability for the management and delivery of care in accordance with established policies and procedures.
