Hospice Nurse

Gentiva Hospice is seeking a compassionate and skilled Hospice Registered Nurse to join our team. As a Hospice RN, you will provide high-quality patient care and support to patients and their families in their homes.
Key Responsibilities:
- Provide direct patient care services to hospice patients and supportive care to the patient's family
- Act as primary liaison between physician, patient/family and hospice team
- Evaluate patient and family needs and assist in the preparation and execution of the plan of care for each patient
- Maintain all assigned patient clinical records
- Participate in company quality assessment and performance improvement programs
Requirements:
- RN license in the state of requested employment (or eligibility to obtain)
- 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology
- Current driver's license and automobile insurance
- Current CPR certification
