Hospice RN Case Manager Position

As a Hospice RN Case Manager at Interim HealthCare Staffing, you will play a vital role in providing exceptional care to patients and families. Your expertise and compassion will make a significant difference in the lives of others.
Key Responsibilities:
- Maintain a specified case management load to ensure timely and effective care.
- Conduct comprehensive assessments of patients and families to determine their unique needs and develop personalized care plans.
- Collaborate with the Interdisciplinary Group (IDG) to ensure seamless care coordination and implementation of care plans.
- Supervise and direct the services provided by LPN/LVN, Hospice Aide, and Homemaker to ensure high-quality care.
- Evaluate and coordinate the need for services, including inpatient, respite, and continuous care, to meet the evolving needs of patients and families.
Requirements:
- Graduate of an accredited school of nursing with a valid RN license in South Carolina.
- At least 12 months of accumulated experience as a registered nurse within the last five years or evidence of current clinical knowledge.
- One year of hospice care experience and one year of precepting or training experience preferred.
- CPR certification and ability to provide proof of valid driver's license and auto liability insurance.
