About the Role

Key Responsibilities
  • Ensure timely spiritual assessments of patients and families/caregivers in the hospice program
  • Facilitate the development of individualized plans of care to meet identified spiritual needs
  • Provide direct spiritual care to patients and families/caregivers
  • Serve as a liaison and support to community chaplains and spiritual care counselors
  • Provide consultation, education, and support to the interdisciplinary group on spiritual care
  • Plan and coordinate funeral or memorial services for patients as requested
  • Actively participate in quality assessment performance improvement teams and activities
Requirements
  • Graduate of an accredited seminary or school of theology (BD, MDiv, or equivalent theological degree) preferred
  • Experience working with death and dying, individuals, families, and caregivers
  • Hospice experience preferred
