Job Summary

Crestwood Behavioral Health is seeking a highly motivated and compassionate Admissions/Discharge Coordinator to join our team. As a key member of our admissions and discharge process, you will play a vital role in ensuring a smooth transition for our clients.

Key Responsibilities:

  • Review and process admission packets, ensuring all necessary information is complete and accurate.
  • Collaborate with conservators and outside sources to facilitate discharge placement.
  • Develop and maintain effective relationships with clients, families, and healthcare professionals.
  • Provide exceptional customer service, responding to client inquiries and concerns in a timely and professional manner.
  • Work closely with the Program Director and Director of Nursing Services to ensure seamless communication and coordination.

Requirements:

  • High School Diploma or equivalent required.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
  • Flexibility and adaptability, with a willingness to learn and grow in a dynamic environment.
