Job Title: Admissions/Discharge Coordinator

Crestwood Behavioral Health is seeking a dedicated and compassionate Admissions/Discharge Coordinator to join our team. As a key member of our care team, you will play a vital role in ensuring a smooth transition for our clients as they enter and exit our programs.

Job Summary:

The Admissions/Discharge Coordinator is responsible for reviewing admission packets, distributing them to the Program Director and Director of Nursing Services for approval, and working with conservators and outside sources for discharge placement. This role requires excellent communication and organizational skills, as well as the ability to manage multiple tasks and priorities in a fast-paced environment.

Responsibilities:
  • Review and process admission packets, ensuring all necessary information is complete and accurate
  • Distribute admission packets to the Program Director and Director of Nursing Services for approval
  • Collaborate with conservators and outside sources to facilitate discharge placement
  • Manage and maintain accurate records and reports related to admissions and discharges
  • Provide exceptional customer service to clients, families, and healthcare professionals
Requirements:
  • High School Diploma or equivalent required
  • Excellent communication and organizational skills
  • Ability to work in a fast-paced environment with multiple priorities
  • Strong attention to detail and accuracy
  • Ability to maintain confidentiality and handle sensitive information
