The successful candidate will be responsible for assisting with the development and maintenance of clinical protocols, collecting data for our Continuous Quality Improvement program, and providing valuable input for day-to-day operations. You will work closely with other healthcare professionals to ensure seamless care coordination and patient outcomes.

Key Responsibilities:

  1. Assist with phlebotomy, immunizations, injections, and health maintenance screenings, following established protocols and procedures.
  2. Provide comprehensive care to patients via phone triage, arranging sick visits as needed, and maintaining a nursing message line.
  3. Review and complete daily tasks assigned by the Nurse Practitioner and clinic manager.
  4. Prepare exam rooms for patient visits, ensuring cleanliness and sterility.
  5. Maintain infection control procedures in accordance with agency guidelines.
  6. Coordinate exchange between the clinic and pharmacy, ensuring efficient flow.
  7. Document all care provided in the electronic medical chart, adhering to standard nursing practice and agency policies.
  8. Process patient referrals for specialty care and follow-up, as needed.
  9. Communicate with internal and external case managers regarding patient care and documentation.
  10. Educate patients on HIV disease, medical care issues, and adherence strategies.

Requirements:

  • Active and unsanctioned license as a Licensed Practical Nurse.
  • Certification in CPR/BLS.
  • Three years or greater experience in a medical setting.
  • Thorough knowledge of nursing practice standards.
  • Ability to be flexible in work hours.
  • Exemplary customer service skills, with strong written and verbal communication abilities.
  • Knowledge of health and social service needs of persons with HIV/AIDS.
  • Must have sensitivity to, interest in, and competence in cultural differences, HIV/AIDS, minority health, sexual practices, chemical dependency, and demonstrated competence in working with diverse communities.

Salary: $48,900 - $61,100
