We are seeking a highly skilled Physician (MD or DO) to join our team at Hospice of Holland Inc. As a key member of our medical staff, you will provide medical care and education to patients and families in a variety of settings, including inpatient, outpatient, home, and facility-based care.
Responsibilities- Provide medical consultation to all Hospice personnel and assume the responsibility of primary physician for patients admitted to Hospice without a qualified primary care physician.
- Participate in admission and re-certification processes and in weekly interdisciplinary team meetings.
- Make visits to patients as appropriate, including acuity, date of admission, pain and symptom management, and regulatory requirements.
- Provide consultation as needed with patient attending physicians regarding symptom control and/or other clinical care plan issues.
- Assist in the Palliative Care program, collaborating on standing protocols, practice standards, and outcome measurements.
- Review patient medical diagnosis, prognosis, medications, procedures, and clinical course to ensure cost-effectiveness and Medicare compliance.
- Develop and maintain effective and positive relationships with healthcare professionals within the community.
- Serve as a source of end-of-life medical expertise, educating and promoting the benefits of hospice care.
- Possess current license (M.D. or D.O.) to practice medicine in the state of Michigan.
- Have board eligibility/certification in internal medicine, family medicine, oncology, emergency medicine, or hospice and palliative medicine.
- Be board certified in palliative medicine or become board certified within 2 years of hire.
- Have practiced medicine for at least three years.
