Key Responsibilities- Assess and Plan Patient Care: Utilize the nursing process to assess patient needs, develop and implement individualized care plans, and evaluate patient progress.
- Provide Direct Patient Care: Administer basic health care and medical treatments, provide wound care, and assist patients with daily living activities.
- Collaborate with Healthcare Team: Work closely with physicians, registered nurses, and other healthcare professionals to coordinate patient care and ensure seamless transitions.
- Document Patient Information: Accurately and thoroughly document patient information, including medical history, treatment plans, and progress notes.
- Maintain a Safe Environment: Ensure a safe and clean environment for patients, staff, and visitors by following infection control protocols and reporting any hazards or concerns.
- Current LVN License: Possess a current and valid Licensed Vocational Nurse (LVN) license in the state of practice.
- Education and Experience: One year of prior LVN experience preferred, with a strong foundation in nursing principles and practices.
- Communication Skills: Excellent communication and interpersonal skills, with the ability to effectively communicate with patients, families, and healthcare team members.
