Nurse Care Specialist

We are seeking a skilled Nurse to join our team at DaVita, a clinical leader in dialysis care. As a Nurse, you will play a vital role in delivering high-quality patient care in an outpatient setting.
About the Role:
This position requires a strong nursing background, with the ability to work effectively in a fast-paced environment. You will be responsible for providing comprehensive care to patients with end-stage renal disease, including administering medications, monitoring vital signs, and educating patients on self-care techniques.
Responsibilities:
- Provide direct patient care, including administering medications and monitoring vital signs
- Develop and implement individualized care plans to meet the unique needs of each patient
- Collaborate with the healthcare team to ensure seamless care transitions and optimal patient outcomes
- Participate in quality improvement initiatives to enhance patient care and satisfaction
Requirements:
- Associates Degree in Nursing (A.D.N) from an accredited school of nursing
- Current RN licensure in the state of practice
- Ability to work effectively in a fast-paced environment
- Strong communication and interpersonal skills
