Job Overview

Position Summary

The Wound Care Specialist Nurse is responsible for delivering exceptional care aimed at preventing and promoting the healing of skin integrity issues among residents, as identified through thorough assessments and personalized care plans.

Key Responsibilities

  • Report any changes in residents' skin integrity to the Director of Nursing or designated personnel.
  • Engage in ongoing training and educational sessions focused on skin care practices.
  • Perform wound dressings and ensure timely changes as per medical directives.
  • Assess and monitor the healing progress of wounds, providing recommendations for adjustments as necessary.
  • Conduct and document weekly evaluations of pressure ulcers, including assessments and measurements.
  • Liaise with family members and attending physicians regarding skin integrity care issues.
  • Maintain documentation on the Weekly Focused Skin Assessment, detailing the outcomes of each resident's evaluation and sharing findings with relevant facility staff during quality assurance and care planning meetings.
  • Assist with additional nursing responsibilities as requested by the Director of Nursing or Administrator, and participate in the facility's nursing on-call rotation.

Qualifications

Educational Background

  • Completion of a nursing program from an accredited institution, whether a college, university, or vocational school.

Licensing Criteria

  • Possession of a valid RN or LVN license in the state of Texas.
  • Wound Care Certification is preferred.

Experience Requirements

  • Prior experience in a skilled nursing environment is advantageous.
