Palliative Care Nurse Practitioner

We are seeking a highly skilled Palliative Care Nurse Practitioner to join our dedicated team at Hospice of the Piedmont Inc. in a full-time capacity. The successful candidate will be responsible for delivering high-quality, patient-centered care to individuals and families facing serious illness and loss.
Key Responsibilities- Palliative Care Visits and Coordination: Provide comprehensive palliative care visits and coordinate care for patients in various settings, including home, assisted living facilities, skilled nursing facilities, and clinic settings.
- Collaboration and Communication: Collaborate with referral sources, primary care physicians, and other healthcare providers to ensure seamless care coordination and quality services for patients and providers.
- Community Outreach and Education: Participate in outreach activities to promote and educate the community about hospice and palliative medicine services, including rotations with nursing and medical students.
- Goal of Care Clarification: Facilitate and document clarification of patient and family goals of care, ensuring that care is aligned with their values and preferences.
- Acute and Chronic Health Condition Management: Assist with diagnosis, treatment, and management of acute and chronic health conditions, in collaboration with the interdisciplinary team.
- Prescriptive Authority: Prescribe medications and order diagnostics/treatments in collaboration with Medical Directors and referring physicians.
- Interdisciplinary Team Collaboration: Participate in internal meetings and provide educational support to the interdisciplinary group, adhering to ethical behavior, professionalism, and confidentiality standards.
- On-Call Responsibilities: Participate in on-call responsibilities and rotation for 24/7 medical coverage, ensuring seamless care delivery.
- Licensure and Certification: Licensed to practice in the state of Virginia, certified as a Clinical Nurse Specialist or Nurse Practitioner, and certification by a national body (ANCC, NBCHPN, AANP).
- Experience and Knowledge: Minimum of five years nursing experience, clinical nursing care in geriatrics, long-term care, and/or hospice/palliative, and knowledge of the interdisciplinary team concept.
- Palliative Care ARN: Palliative care ARN certification is preferred.
