We are seeking a dedicated Palliative Care Nurse Specialist to provide exceptional care and support to patients facing serious health challenges. This role is integral to our mission at Hospice of the Piedmont, where we prioritize compassion and respect in our palliative care services.
Key Responsibilities:- Conduct palliative care visits and manage patient care across various environments, including home and healthcare facilities.
- Work collaboratively with healthcare providers and referral sources to ensure optimal service delivery.
- Engage in community outreach to enhance awareness and understanding of hospice and palliative care.
- Facilitate discussions to clarify patient and family care goals.
- Support the diagnosis, treatment, and management of both acute and chronic conditions.
- Participate in educational initiatives with nursing and medical students.
- Consult with the healthcare team to develop and implement comprehensive care plans.
- Prescribe necessary medications and order diagnostic tests in partnership with medical directors.
- Contribute to team meetings and provide educational resources to the interdisciplinary team.
- Uphold ethical standards and maintain confidentiality in all interactions.
- Participate in on-call duties to ensure continuous medical support.
- Valid nursing license in the state of Virginia.
- Certification as a Clinical Nurse Specialist or Nurse Practitioner.
- National certification (ANCC, NBCHPN, AANP) is required.
- A minimum of five years of nursing experience, particularly in geriatrics, long-term care, or hospice/palliative settings.
- Palliative care certification is preferred.
- Familiarity with the interdisciplinary team approach is essential.
