Overview:
The Anthony L Jordan Health Corporation, a pioneer in health services since 1904, is one of the first Federally Qualified Health Centers (FQHC) in the nation. Our mission is to provide essential health care services to individuals facing barriers to access, ensuring a comprehensive approach that includes medical, dental, behavioral health, and community services.
Role Summary:
We are seeking an Advanced Practice Provider (APP) who will work under the supervision of the Chief Medical Officer. The APP will be an integral part of a medical team, delivering holistic care and education to patients and their families. This role involves assisting families in navigating community resources and reducing barriers to care through effective support systems. The APP will also coordinate services for high-risk families with multiple challenges.
Key Responsibilities:
- Provide confidential and comprehensive health care services with respect and empathy.
- Deliver the highest quality of patient-centered care.
- Promote the physical, social, and emotional well-being of patients and their families.
- Enhance patient access to necessary care.
Qualifications:
The ideal candidate will possess:
- A completed advanced educational program recognized by the New York State Education Department, equipping them to provide primary health care services.
- Certification or eligibility for certification in a specialty that includes primary health care services.
- A current New York State license.
- Knowledge and skills demonstrated through certification as a Physician Assistant or equivalent within two years of employment.
Special Skills:
The candidate should exhibit:
- Strong interpersonal skills to effectively engage with patients.
- Ability to deliver nursing care through health promotion, technical expertise, and crisis intervention.
